Response Performance Report
Want to keep track of specific inspections or survey responses?
By running the Response Performance Report, you can easily keep track of the responses selected during surveys or inspections. It breaks down the responses selected by time (month & day of week), location, topic, evaluator, and operator, making it easy to spot trends and compare performance.
Note: Only Account Administrators & Managers can generate reports.
When signed in to the system, click on the Reports tab in the left-hand menu bar. Once the menu is open, click on the Performance - Response report.
You are required to select an Inspection Template for this report.
Note: you can select multiple Inspection Templates by selecting CTRL on your keyboard and clicking on the inspection templates at the same time.
There are 6 versions of this report that you can view by using the tabs at the bottom of the report screen.
a.Month – Displays how responses vary over time to help identify trends and month-to-month performance.
b. Day of Week – Shows how responses are distributed across weekdays to highlight any day-specific patterns or inconsistencies.
c. Location– Compares responses across locations.
d. Topic– Compares responses across Inspection Topics (Bed, Floor, Door, etc.)
e. Evaluator - Compares responses across Evaluators.
f. Operator – Compares responses across Operators.
Note : The title of the report will indicate the tab you are viewing.
Responses by Month
a. Average Score Gauge - shows the average inspection score for the selected time period. The target is 90%, and the current score is 86%.
b. Response Option Color Legend
🔴 Red Bar = "Fail" Responses
🟢 Green Bar = "Pass" Responses
🔷 Blue Diamond = Total number of Responses
(Note: the color and text of the responses will match those of the inspection template selected.)
c. Response Rate (left axis) - the percentage of each response for each month.
d. Count of Responses (right axis) - the total number of responses completed per month.
e. Monthly Responses Breakdown - bars show the distribution of response types each month, helping identify trends or changes over time.
f. Timeline (months) - shows which months are included in the report, allowing for easy month-to-month comparison.
Responses by Day of Week
a. Average Score Gauge - shows the average inspection score for the selected time period. The goal is 90%, and the current score is 78%.
b. Color and Symbol Legend -
🔴 Red Bar = "Fail" Responses
🟢 Green Bar = "Pass" Responses
🔷 Blue Diamond = Total number of Responses
(Note: the color and text of the responses will match those of the inspection template selected.)
c. Response Rate (left axis) - the percentage of each response per day of the week.
d. Count of Responses (right axis) - the total number of responses completed per day of the week.
e. Day of Week Breakdown - each grouped bar shows how responses were distributed for each day of the week, making it easy to spot which days had stronger or weaker performance.
f. Day of Week (bottom axis) - days of the week, so you can clearly compare performance from Sunday to Saturday.
Responses by Location
a. Average Score Gauge - shows the average inspection score for the selected time period. The goal is 90%, and the current score is 82%.
b. Color and Symbol Legend -

🟢 Green Bar = "Pass" Responses
🔷 Blue Diamond = Total number of Responses

c. Response Rate (left axis) - the percentage of each response per location.
d. Count of Responses (right axis) - the total number of responses completed per location.
e. Location Breakdown - each grouped bar shows how responses were distributed for location, making it easy to spot which locations had stronger or weaker performance.
f. Location (bottom axis) - the names of each location (e.g., "Main Campus"), so you can see how each location is performing.
The Campus will be displayed by default. You can drill down further into each location node (Building-> Floor->Area->Room) by clicking right on the bar graph and then clicking Drill Down. Repeat until you get to the location node you wish to view.
Responses by Topic
a. Average Score Gauge - shows the average inspection score for the selected time period. The goal is 90%, and the current score is 82%.
b. Color and Symbol Legend -
🔴 Red Bar = "Fail" Responses
🟢 Green Bar = "Pass" Responses
🔷 Blue Diamond = Total number of Responses

c. Response Rate (left axis) - the percentage of each response per Topic (i.e., Curtain - Privacy, Toilet, Sink, etc.)
d. Count of Responses (right axis) - the total number of responses completed per Topic.
e. Topic Breakdown - each grouped bar represents how responses were distributed for specific inspection topics (e.g., Curtain - Privacy, Toilet, Sink). This helps identify which topics consistently meet standards and which may need attention.
f. Topic Names (bottom axis) - lists the Topics included in the report so results can be viewed and compared by Topic.
Responses by Evaluator
a. Average Score Gauge - shows the average inspection score for the selected time period. The goal is 90%, and the current score is 82%.

f. Evaluator Names (bottom axis) - lists the evaluators included in the report so results can be viewed and compared by person.
Responses by Operator

f. Operator Names (bottom axis) - lists the Operators included in the report so results can be viewed and compared by person.
To export the report, click the "..." icon at the top right-hand corner of the report table and then click on Export Data.
- To open the report in Full-Screen, click on the arrows at the top right-hand corner of the report.
To focus/expand on the report graph, click the Focus Mode icon.
- By clicking on the blue View Filters Selected button on the blue left-hand menu, you can filter on the following parameters:
a. Date Range
b. Inspection Template (Mandatory)
c. Evaluator
d. Operator
e. Assignment
f. Inspection Template (selected in "b" or see step 2)
g. Location
h. Location Model (i.e., patient room, restroom, O.R., etc.)
The report data will update automatically as the filters are applied.
